Adoption Application

Lucky Day Animal Rescue Adoption Application Fees

Every Lucky Day animal is current on all vaccinations and spayed or neutered. Puppies and kittens are wormed. Puppies and kittens under 6 months of age may not have been neutered or had their rabies shots yet. They will often also need to have their vaccinations boosted, especially if they are under 16 weeks of age. This will be the responsibility of the adopting home should they be due for a booster or rabies shot after they are adopted.

Adoption Process
We require that you sign a contract with us, promising that you will give the animal proper food, exercise and medical care among other things, and that should you ever have to surrender the animal (which we certainly hope you will not), that it must be returned to Lucky Day or a Lucky Day approved rescue group or home.

Sorry, we do not adopt out of state (outside Colorado).

BEFORE completing this application, we strongly recommend that you:

  1. Take a moment and view this powerful 3-minute online video, “The Apology”, on pet ownership awareness. We invite everyone to consider the video’s factors when thinking about adding a pet to their family!
  2. Click here to read an article about the cost of dog ownership.
  3. Read “Things to Consider Before Adopting.”

Thank you!

We apologize in advance if you are bounced off while trying to complete the form below (it seems to have problems with certain devices such as iPads). If you are bounced off, please contact us directly via phone, 970.379.4606, or email, info@luckydayrescue.org.
